General
The AVG is the law to protect privacy and personal data. Under this law, an organization that works with personal data has certain duties and the person whose data belongs has certain rights. You should keep in mind that medical records under the law are basically kept for a maximum of 20 years. You help the Paramedic Naturopathic Therapist with looking up your file and protecting your privacy if you describe what you want to request as completely as possible. The information you provide will be kept strictly confidential by the Paramedic Naturopathic Therapist. The Paramedic Naturopathic Therapist is not liable in mail delivery. If you prefer to pick up the file in person, or by an authorized representative, please indicate this in your letter/orally.
Patient data
You include the details of the person about whom the medical record is about. The Medical Contract Act (WBGO) considers the patient to be of age from 16 years of age. Adolescents from 16 years of age who want to inspect/remember their medical records must make the request themselves. If the patient is no longer alive, provision of the medical records is permitted if it can be assumed that the deceased would not have objected or there are compelling interests to break the healthcare provider's duty of silence. This decision lies with the healthcare provider.
Provision of your personal data to third parties
The Paramedic Naturopathic Therapist has an obligation to treat your personal data confidentially. This means, for example, that the Paramedic Naturopathic Therapist requires your explicit consent before providing your personal data. However, there are some exceptions to this rule. On the basis of a legal requirement, the obligation of silence of the care provider can be broken, but also when there is a fear of a serious danger to your health or that of a third party. Moreover, if necessary, recorded data can be exchanged orally, in writing or digitally with other healthcare providers (e.g. the pharmacist who processes a prescription or your general practitioner/dentist).
Data exchange
The Paramedic Naturopathic Therapist does not exchange data with third parties without your permission. Only with your permission will information, e.g. with your treating physician/dental practitioner be exchanged. Data may also be shared (but only with your permission) with your pharmacy and your treating medical specialists. This includes the medication prescribed to you by the general practitioner/dental practitioner, as well as any intolerances, contraindications and allergies (ICA data). Transfer of your file
If you choose another practitioner, it is important that your new practitioner is aware of your Naturopathic/Medical history. Your history is in your client file. It is customary for your old practitioner to transfer the file to your new practitioner. The old practitioner will do this (at your request) as soon as possible, at least within a month, after you have asked the Paramedic Naturopathic Therapist to transfer the file to your new practitioner.
Your file will then be transferred by your practitioner in person or by registered mail. You cannot take the original file with you. However, you are always entitled to inspect your file and to receive a copy of your file. The file can (if possible) also be transferred to the new practitioner by e-mail. Both practitioners must ensure that their computers and internet connections are sufficiently secure.
Important
You may give written permission to the Paramedic Naturopathic Therapist to receive your treatment invoice in pdf over your email, or that you wish to receive it by mail. (Form is present at first consultation).
The invoice will include your name address, date of birth, health insurance, policy number and invoice amount.
Procedures to protect your personal data and privacy during consultation, transportation from theft and loss
If you wish to make an appointment for a consultation you must do so by telephone. During the consultation your personal data will be recorded. You can exchange treatment data with the Paramedic Naturopathic Therapist by secure email. If desired you can contact the therapist by phone about treatment data. The Paramedic Naturopathic Therapist is not liable for the safety of the telephone line during the exchange of personal and/or treatment information by phone.
Your entire client file and personal information are securely stored in a locked cabinet and are inaccessible to unauthorized persons.
At the first consultation you are required to sign a treatment agreement in which you agree to accept the General Terms and Conditions of 2theCore Naturopathic Therapist diens compliance with the AGV and WGBO laws (see www.2theCore.nl) and the recording of your personal and treatment data in your client file at 2theCore Naturopathic Therapist. These data consist of: name, address, place of residence, date of birth, policy number, health insurer, email addresses, phone numbers. In addition, treatment data such as; x Rays, blood tests and other information that is important for the treatment plan of you as a client and noted and or obtained with/by client's consent.
